Medicare Cut the Price of Obesity Drugs to $50 and Having Diabetes Disqualifies You

Medicare will sell you a GLP-1 medication for $50 a month. It will not sell it to you at that price if you have type 2 diabetes or moderate to severe sleep apnea. About 5.9 million people are on the wrong side of that line.

How the Program Works

The Medicare GLP-1 Bridge launched in July 2026 as an 18 month pilot. Eligible Part D enrollees pay $50 a month for Wegovy, the Zepbound KwikPen, or the oral drug Foundayo.

Eligibility runs on body mass index. A BMI of 35 or higher qualifies on weight loss alone. A BMI between 27 and 34 qualifies if the patient also has a condition such as prediabetes or cardiovascular disease. Part D enrollment is required either way.

The Rule That Sends the Sickest Patients Back

The $50 price applies only when the drug is prescribed solely for weight loss. Anyone with an FDA approved medical indication for a GLP-1, which includes type 2 diabetes and moderate to severe obstructive sleep apnea, is routed back to their regular Part D plan.

That plan can require copays of hundreds of dollars a month. KFF Health News puts typical Part D copays for these drugs at $200 to $600 a month, if not more.

What That Means at the Counter

About 3.8 million Medicare beneficiaries are estimated to qualify for the $50 price. About 5.9 million overweight enrollees are excluded because they already have a qualifying medical condition.

Two people can fill the same prescription at the same pharmacy on the same day and pay $50 or $600, and the one paying more is the one with the additional diagnosis.

The Rule Is Working as Written

This is not an administrative error. The demonstration was designed to cover weight loss, which standard Medicare does not, and to avoid duplicating coverage that already exists for approved medical indications.

The effect is still that a diagnosis which makes a patient sicker also makes the drug more expensive. CMS spokesperson Timothy Foster said the program has allowed thousands of eligible beneficiaries to access GLP-1 medications for weight loss at pharmacies nationwide. The pilot runs 18 months from July 2026, and anyone excluded is paying the difference for the duration.
