New Medicare Advantage Flex Card Rules Could Trigger Checkout Declines in 2027

Some Medicare Advantage debit-card purchases could be declined at the register in 2027 if the item is not covered by the member’s plan. The change comes from a CMS rule requiring these cards to verify plan-covered items and services in real time at the point of sale.

The rule does not mean every Medicare flex card will suddenly stop working. It is designed to match each purchase to the supplemental benefit the member’s plan actually covers.

What CMS Is Changing

CMS finalized requirements for debit cards used to administer Medicare Advantage supplemental benefits. The cards must be electronically linked to plan-covered items and services through a real-time identification mechanism.

In practical terms, the eligibility check moves into the transaction itself. The system is supposed to determine whether the item or service is eligible for the specific benefit before the purchase is approved.

CMS also says the cards must be limited to the specific plan year. The goal is to make it clearer what the supplemental-benefit dollars are being used for and to reduce improper spending.

Why a Purchase Could Be Declined

A member may have a card with money available on it and still encounter a decline if the product in the cart is outside the plan’s covered benefit. That does not necessarily mean the card is broken or the entire allowance has disappeared.

The covered-item list can vary by plan and by benefit. A card used for over-the-counter health products, healthy foods or another supplemental category must still follow the terms of that particular Medicare Advantage plan.

Mixed purchases can also make the checkout experience more noticeable because covered and non-covered items may need to be separated or handled differently by the retailer’s payment system.

What Members Should Compare During Enrollment

The face value of a supplemental allowance is only one part of the benefit. Members should also look at which products qualify, which retailers participate, how often the allowance reloads and what happens when a card transaction does not work as expected.

That makes two plans with similar dollar allowances potentially very different in actual use. A smaller allowance with a broader eligible-item list may be more useful to one person than a larger allowance with tighter restrictions.

What Changes in 2027

The rule applies to 2027 Medicare Advantage plans. Members comparing plans during the fall enrollment season should read the plan’s evidence of coverage and supplemental-benefit materials rather than assuming a flex card will work exactly as it did in 2026.

The change does not mean every flex card will be declined. It means the checkout system will play a bigger role in enforcing what the plan says the card can buy.
